    Five years ago, I took a career risk by leaving my job to work on a ship. My medical friends did their best to persuade me, saying that running away to sea would ruin my career. But after these years working as a junior, I was willing to take the risk.
    Hungry for adventure, I boarded a ship in Singapore with 2,000 passengers and crew. To my relief, the hospital was well equipped, with an X-ray machine and a blood analyzer. That first voyage was a learning experience, a tight schedule full of safety drills. There was so much new information to take in. Even remembering which uniform to wear each day was a challenge. Most confusing, I often forgot to set my clock when the ship crossed time zones.
    As a doctor, I was responsible for 600 crew, and I was on call for the entire ship. Far from treating seasickness and sunburn, I had to deal with other diseases, for my patients were wide and varied. The ship's medical center was actually a floating emergency room, and we didn't have a team of specialists on hand for a second opinion. With long and unpredictable hours, it required mental toughness.
    As you can guess, many of the passengers were elderly, heart attacks don't care about geography and emergency evacuating(疏散)was difficult to arrange. I recall one such patient, who was taken off the ship halfway through the Panama Canal. After a terrifying ride in an old ambulance, I was relieved that the patient survived long enough to arrive at the hospital in Panama City.
    Thankfully, there were several unexpected benefits to the job, I regularly enjoyed the passenger facilities and I even hosted my own table of passengers in the evenings. On rare days off, I volunteered as a tour guide on trips ashore. I got to fly over Alaska in a seaplane and watched a ballet in St. Petersburg.
    Now, I understand being a ship doctor is not a job—it's a way of life. One year at sea became two. I lost my career ambitions, but I redefined happiness in my life.

    “ New and improved. ” These words are put in so many marketing campaigns that we tend to accept them as linked. But many new drugs aren't an improvement over the best existing drug for a given condition, and the fast drug-approval processes in recent years have added to the uncertainty about their advantages.
    A recent report in the British Medical Journal, "New Drugs: Where Did We Go Wrong and What Can We Do Better?”analysed the issue, The authors looked at 216 drugs approved between 2011 and 2017 ;152 were newly developed, and 64 were existing medicine approved for new uses. Only 25% offered a major advantage over the established treatment, and fully 58% had no confirmed added benefit to reduce symptoms or improve health-related quality of life.
    "This doesn’t mean there's no added benefit," lead author Wieseler said. “It just means we have no positive proof. Either we have no studies or have studies not good enough. ” Wieseler and her co-authors work for a German institute which evaluates new treatments and advises on whether the country's health care system should pay a premium ( 附加费)for them. Such organizations , known as health technology assessment ( HTA) agencies, work a little differently in the US, says Sean Tunis,a researcher in Baltimore: “ If payers think a new drug isn't better than an existing drug,these agencies will require that hospitals try the cheaper drug first. ”
    Germany's HTA demands trials to prove that a new treatment beats the existing standard. This isn't always practical For one thing, such studies can be expensive and time-consuming, with no guarantee of success. Secondly, it can discourage companies from attempting to develop new alternatives. This is already happening. Drug developers are increasingly focused on areas where there are no good treatments to compete with, such as rare diseases.
    This lack of meaningful data to guide patients is a major point of Wieseler's paper- With accelerated approval, there are more products approved, with a greater amount of uncertainty about risks and benefits. But there are other solutions besides drug trials. One idea is to require postmarket studies to track the effectiveness of newly approved drugs—a step too often neglected.

    China’s war on garbage is as digital as the country itself. Think QR codes attached to trash bags that allow the government to trace exactly where its trash comes from.
    On July 1, Shanghai began a compulsory garbage sorting program. Households and companies must classify their wastes into four categories and dump them in appointed places at certain times. The strict program became a headache for some residents. Not even the most environmentally conscious person can get all the answers right. Like, which bin does the newspaper you just used to pick up dog poop (粪便)belong to?
    Gratefully, China’s tech startups are here to help. For instance, China’s biggest internet companies responded with new search features that help people identify what wastes are “wet”, “dry”, “toxic”, or “recyclable”. Simply pull up a mini app on WeChat, Baidu or Alipay and enter the keyword. The tech firms will give you the answer and why.
    Alipay, Alibaba’s electronics payment company, claims its garbage sorting mini app added one million users under just three days. The mini app has so far indexed (编索引) more than 4,000 types of rubbish. Its database is still growing, and soon it will save people from typing by using image recognition to classify trash when they snap a photo of it. If people are too busy or lazy to hit the collection schedule, well, startups are offering trash service at the doorstep. A third-party developer helped Alipay build a recycling mini app and is now collecting garbage from 8,000 apartment complexes across 11 cities. To date, two million people have sold recyclable materials through its platform.
    Besides helping households out, companies are also building software to make property managers, life easier. Some residential complexes in Shanghai began using QR codes to trace the origin of garbage. This way, regulators in the region know exactly which family has produced the trash and fine violators.
