Since the Institute f Medicine (IOM) reprt in 1999 “T Err is Human,” an increasing number f studies have shwn hw cmmn and deleterius ME are, especially in hspital medicine. With this, healthcare leaders invested time and resurces tward identifying and reducing ME.
A medical errr is defined as “an incidence when there is an missin r a mistake in planning r executin that leads r culd lead t unintended result.” While the majrity f ME d nt lead t an apparent adverse effect, a significant number f patients either suffer a permanent injury r death frm ME every year in the United States and arund the wrld as a result f thse errrs.
